Overhead pain is rarely a single structure failing. It is usually a sequence: the mid-back does not extend, so the shoulder blade cannot rotate up, so the rotator cuff works at a mechanical disadvantage at exactly the range where you need it most. Treat the sore tendon and ignore the sequence and you get temporary relief.

What We See Most

Where the shoulder tends to break down.

A painful arc in the middle of raising the arm that eases at the top. Pain that wakes you when you roll onto that side. Weakness reaching behind you or into a back seat. A shoulder that clicks without pain — usually irrelevant — versus one that catches and stops, which is not.

Among athletes: pain at the bottom of a bench press, at the catch of a snatch or a kipping pull-up, or late in a throwing session. Each points somewhere different, which is why the question is always what you were doing when it started.

The sequence, not the spot

Thoracic extension and rotation, then how the shoulder blade moves as the arm goes up. A stiff mid-back changes what the cuff has to do at every degree above shoulder height.

Strength tested at the positions that actually hurt, not only the comfortable mid-range ones where almost everyone tests strong.

Your movement, loaded

The press, the pull-up, the throw, the reach overhead at work. We watch the thing that hurts rather than asking you to describe it.

Symmetry in numbers

VALD upper body testing measures force production and side-to-side difference, which matters when you are returning to overhead sport or recovering from surgery.

Treatment

Open the range, then earn the right to use it.

Hands-on work comes first when range is the limiter: joint mobilization for the thoracic spine and shoulder, and dry needling where cuff or upper trap tone is holding the position hostage. That part often feels dramatic and is the easy half.

The half that lasts is loading the new range so your body keeps it. For post-operative shoulders and irritable tendons that will not take heavy weight yet, blood flow restriction training keeps strength moving in the meantime.

Common questions

My scan showed a rotator cuff tear. Do I need surgery?

Not automatically. Cuff tears are common in people with no symptoms at all, and many degenerative tears do well with loading rather than repair. Traumatic tears, significant weakness, and some younger patients are a different conversation. We are not the ones who make that call, but we can tell you what your shoulder is currently capable of and coordinate with your surgeon.

Why does it hurt more at night?

Common with cuff-related pain: side-lying compresses the area and there is nothing else competing for your attention. It is a recognised pattern rather than a sign of something sinister, though it is worth mentioning at your evaluation because it shapes early treatment.

Can I keep pressing and pulling?

Usually, with changes. Range, grip, tempo, and volume are all adjustable before "stop" becomes the answer. Kipping and heavy overhead work may pause for a stretch — we will tell you roughly how long rather than leaving it open-ended.

How long is frozen shoulder?

Genuine adhesive capsulitis runs on its own timeline, often many months, and no clinic can shorten it dramatically. What treatment does change is how much range and strength you keep along the way and how uncomfortable the middle stretch is.

Do I need a referral for physical therapy in Texas?
