Sometimes called scraping or tool work. We use a heated stainless steel instrument, drawn along restricted tissue to improve how it glides — and, just as usefully, to find where the restriction actually is.

Hands are excellent at treating tissue and limited at feeling fine texture through their own soft tissue. A rigid instrument transmits what is underneath far more clearly, which is why IASTM is as much an assessment tool as a treatment one.
What It Is
A tool that sharpens what the therapist can feel.
The instrument is drawn along the tissue with an emollient. Restricted areas transmit a distinctly different feedback through the steel than healthy tissue does, so the therapist can map the restriction rather than working from where you point.
The mechanism is less exciting than the marketing suggests. The old story about "breaking down scar tissue" does not hold up — the forces involved are nowhere near what that would require. The current understanding is that it works through the nervous system and local circulation: changing how the tissue is perceived and how it tolerates being loaded.
That distinction matters practically. Because the effect is largely neurological and temporary, it only pays off if you use the window it opens — which is why IASTM here is always followed by loading in the range you just gained.
Our instrument is heated. Warm tissue is more comfortable to work on and tolerates pressure better, which in practice means the same treatment can be done with a lighter hand — less discomfort during the session and less soreness and marking afterwards. It is a comfort and tolerance advantage rather than a different mechanism; the heat does not do anything to the tissue that the instrument alone could not.

What to Expect
Firm pressure, short treatment, then work
Warm from the first pass
The instrument is already heated when it reaches you, so there is no separate warm-up step. Most people describe the first contact as closer to a hot stone than to cold steel.
Some redness is normal
Mild redness in the treated area is expected and fades quickly. Significant bruising is not a goal, not a sign it worked, and not something we chase.
Then you load it
The session moves straight into using the range through movement and resistance. That is the part that makes any of it last past the afternoon.

IASTM sits alongside the other hands-on options rather than competing with them. Cupping decompresses tissue rather than compressing it and tends to suit broader areas and general recovery. Dry needling reaches deeper and addresses muscle tone more directly. Joint mobilization is the answer when the restriction is the joint rather than the tissue over it.
Which one you get depends on what the assessment finds, not on what the clinic prefers to sell.

FAQs
Common questions
Does it break up scar tissue?
No, and anyone telling you otherwise is repeating an outdated explanation. The forces a handheld instrument can generate are far below what would be needed to remodel scar tissue mechanically. The effect appears to be neurological and circulatory — real, but not that mechanism.
Should it leave bruises?
No. Mild transient redness is normal; heavy bruising means too much pressure for too long. More aggressive is not more effective, and a bruised area is one you cannot load well afterwards, which defeats the purpose.
Why is the IASTM instrument heated?
Comfort and tolerance. Warm tissue takes pressure better than cold tissue does, so the same work can be done with a lighter hand — which usually means less discomfort during the session and less soreness and marking afterwards. The heat is not doing something separate to the tissue; it makes the treatment easier to receive, and a patient who is not bracing is a patient we can actually treat.
Does it hurt?
Firm and sometimes tender over a restricted area, but it should stay tolerable — the heat takes a good deal of the edge off compared with a cold instrument. Expect possible mild soreness for a day afterwards, similar to after a hard training session.
How is this different from cupping?
Opposite directions. IASTM compresses and shears tissue with a heated instrument; cupping decompresses it with suction and red light. IASTM tends to suit specific, localised restriction; cupping suits broader areas and recovery.
Can I buy a tool and do this myself?
You can buy one, and plenty of people do — though consumer tools are not heated, so you are working on cold tissue and will need more pressure for less comfort. What you cannot replicate at all is knowing where to work and how long to spend: most of the value is in the assessment behind it, and the main risk of doing it yourself is too much pressure for too long on the wrong area.
